PLEASE NOTE: Children between birth and 36 months old may be eligible for AzEIP. If the child you wish to refer has already turned 3 years old, you may seek services through your local school district. Please go to AZ Find for further information.

If you refer a child using this link, the information you submit will be forwarded by email to your local AzEIP office, which will initiate contact with the child and family. If you are not the parent or guardian, they will be contacted for permission to proceed with your referral, and they may accept or decline.
